A $25 charge may be applied for a missed appointment

A security deposit or deposit option is necessary to activate natural gas service.
You may choose one of the following options to secure your natural gas account.
  I will pay a deposit at the time my turn on order is scheduled.
The deposit is based on twice the monthly consumption and is used to secure your account. The deposit will accrue interest that is applied to your account and will be refunded after 23 months of continuous service and prompt payment.
  I would like to use my history on the previous Peoples Gas account listed above to waive my deposit.
The account must have been active within the last six months and must display one year of continuous service with no late payments or returned checks.
I understand that until the option I have selected is approved, the deposit charge will remain on my account. The deposit or a deposit option will be required before activating service.
